Offer terms: $39 is a one-time initial visit for new self-pay patients and includes a history, exam, and a plain-English plan explained before any care begins. It is not a membership. $89/month membership is optional; visit allowance and inclusions are confirmed in writing at signup. Offers are self-pay only and are not combined with insurance, PIP or MedPay billing. Ask us about eligibility. Membership may be cancelled with notice under the membership terms you receive at signup. Prices can change; the price confirmed at booking applies.
